Uses the nursing process within a primary care
nursing model. Functions effectively in accordance with

Specifications:
1. Degree
in nursing from an accredited program.
2. License to practice as a registered nurse in the State
of Michigan.
3. Knowledge of and ability to apply Community Health
Nursing principles and practices.
4. Ability to travel within the assigned community.
5. Strong clinical and communication skills.
6. Ability to exercise independent judgment.
7. Willingness to accept philosophy, purpose, and ideals
of the agency.

| POSITION
TITLE: Licensed Practical Nurse |
| Position
Summary: The Licensed Practical Nurse (LPN) provides
basic nursing care to patients in their home. The LPN
carries out assigned duties under the direction or supervision
of a Registered Nurse. The LPN exercises limited
independent judgment with the nursing care plan. Each LPN
performs nursing services for residents within the service
area. Services are performed in accordance with the
physician's orders and under the direction and supervision of
the Registered Nurse and the Director of Professional Services. |
| Qualifications:
A. Currently
licensed as a Licensed Practical Nurse in the State of Michigan
or has a Temporary Graduate Nurse License.
B. Must have a negative T.B. skin test or chest
x-ray. |

| POSITION
TITLE: Home Health Aide |
| Position
Summary: The Home Health Aide provides temporary
supportive service to compliment the Community Health Nurse or
Therapist's efforts to resolve the immediate needs of the
patient and family. The scope of the Home Health Aide
services will be determined, directed and supervised by the
professional staff. |
| Position
Specifications:
1. Minimum
of high school education.
2. Documented evidence of completion of the state
certified Home Health Aide training program.
3. Ability to interpret directions for care and proceed
in carrying out written and verbal orders given by supervision.
4. Ability to accurately report written and verbal
information regarding patient care.
5. Ability to effectively handle the physical demands of
the position on a daily basis.
6. Ability to be compassionate and understanding of
patients and family needs.
